Application and Other Required Forms

We are pleased to know that you are considering High Meadows School for your child. To make the process as easy as possible, we have organized our admission forms for you here:


PS-3, PreK-4 and Kindergarten Admission Forms

  • Please complete the Admission Application Download a 2012-2013 Application
  • If your child is currently in school elsewhere, please have your child's current teacher fill out a Teacher Evaluation form (download).
  • All completed forms and $100 non-refundable application fee per child are due to High Meadows School. Upon receipt of these materials, we will contact you to schedule a one-hour Play Assessment for preschool applicants (no parent-child separation), and a half-day visit to a PreK-4 classroom and academic assessment for Kindergarten applicants.

 

First through Fifth Grade Admission Forms

  • Please complete the Admission Application Download a 2012-2013 Application
  • First - Fourth Grade: Please have your child's current school complete the AAAIS Release of Information form (download) and have your child's current teacher complete the AAAIS Teacher Evaluation form (download).
  • Fifth Grade: Please have your child's current teacher complete the AAAIS Teacher Evaluation form (download); have your child's current school complete the AAAIS Release of Information form (download); and have the principal or counselor at your child's current school complete the AAAIS Principal/Counselor Evaluation form (download).
  • All completed forms and $100 non-refundable application fee per child are due to High Meadows School. Upon receipt of these materials, we will contact you to schedule a one-day classroom visit and academic assessment.

 

Middle Years (6th, 7th, 8th Grade) Admission Forms

  • Please complete the Admission Application Download a 2012-2013 Application
  • Please have your child's current teacher complete the AAAIS Teacher Evaluation form (download); have your child's current school complete the AAAIS Release of Information form (download); and have the principal or counselor at your child's current school complete the AAAIS Principal/Counselor Evaluation form (download)
